02 / A little perspective
Big day.
Small mercy.
A small mercy is something that makes a hard day a little easier. Ours starts with the paperwork.
Why Small Mercy ↗There’s still a person
behind the note.
And a life waiting beyond it.
For the conversation in front of you.
And the rest of your day.
Paperwork, meet your match.
You talk with your patient; Small Mercy listens. On paid plans, words stream as you speak. Who said what is settled after the consult, not guessed live. The note drafts itself from only what was said.
Start a consented session. On paid plans, Small Mercy transcribes in near real time, then tells your voice from everyone else’s once capture ends — when it’s unsure, it says so.
It structures the note — SOAP or your template — from only what was said, flagging anything uncertain and marking the absent as [not stated].
Re-read, correct and, on paid plans, listen back. Every statement is grounded to the transcript. Nothing saves itself.
Confirm the reviewed note. Free deletes audio when the draft arrives; paid deletes on confirmation by default. Your patient can keep a QR deletion receipt.
Fluent doesn’t mean flawless.
You are responsible for the note you confirm. Small Mercy shows its doubt: it flags what it isn't sure it heard and what it couldn't place, so your attention goes straight to the few spots that need it most — and you still read and confirm every note.
"Not sure I heard this." On paid plans, a dotted underline lets you listen back. Free marks the line to re-read, with no audio playback.
"Heard it, couldn't place it." A highlighted block with a review tag — re-read and settle it.
[not stated] — anything absent is marked, never invented. Safety-critical gaps are flagged, not silently dropped.
Only what was said.

Private by design. Small Mercy processing in Australia.
Clinical records in Sydney. Default transcription and drafting on vetted Australian services. Clinical data is never used to train models.
Read the processing details
On AU Sovereign, Small Mercy stores the clinical record in Sydney and runs transcription and note drafting on vetted Australian services over private links — disclosed on every note under your organisation’s processing consent. The optional Global note fallback is currently disabled; if enabled, after an in-cell note-drafting failure a clinician may accept a single-use offer to send only that consult’s transcript for its note and later redrafts, never audio or other outputs. Global processing may occur outside Australia; the organisation and session stay AU Sovereign, with no automatic switch. Consent to record is separate for each consultation. Speaker identity is matched only on Small Mercy’s infrastructure. A Small Mercy-managed route outside Australia needs separate, explicit acceptance for that session and purpose. Sending a confirmed note or letter to connected practice software is a separate clinician action into the vendor account; Nookal’s Australian API service alone does not establish where that account stores data. Audio is deleted by default and clinical data is never used to train models. The patient can see the consult consent and processing record.

Built for the way clinics actually run.
Built for allied health too — physiotherapy, chiropractic, podiatry and psychology as much as general practice. The loop is the same whatever you practise: it listens, drafts, and waits for you to confirm. The clinical tuning underneath it is Australian and GP-shaped today, and the consult-note and letter workflows do not yet have discipline-specific formats — we'd rather say so than imply tuning we haven't built. Two-party consults. Your words and the patient's, kept clearly apart.
Bedside to bedside — phone capture, or wrist capture on paid plans.
Minutes, decisions and dated action items — confirmed before they're saved.
Letters and notes by voice, with RACGP-aware referral drafting.
Paid desktop capture for both sides of a Zoom or Teams consult — on Windows and on Mac.
Works alongside any practice system today — copy the confirmed note in one click. On paid plans, Small Mercy also connects directly to Cliniko for Australian-hosted accounts and Nookal through its Australian API service: start the consult from today’s booking, then file the confirmed note back as a draft for you to finalise. Confirm your Nookal account’s storage region with the vendor. More on integrations →
Numbers we're prepared to stand behind.
Historical internal tests, with the engine and dataset named. These results do not measure today’s managed pipeline or predict performance in your clinic.
See our internal benchmarks +
Historical June 2026 Deepgram Nova-3-medical reference, without drug keyterm boosting: 1 fabricated medication name in the same 158 clips; 11 clips were empty or dropped. This historical run does not establish current production performance.
Historical June 2026 DeBERTa-base calibration: grounded-pair precision 1.00 on 40 labelled pairs, also used to select the threshold. This is not held-out validation or a percentage of correct notes.
The historical July 2026 raw self-hosted ReDimNet2-b6 attribution configuration recorded zero confident-wrong speaker labels at 80% recall. Other tested configurations did not; this is not a result for managed anonymous diarisation.
The benchmarks are Australian because the product is. It's tested on Australian-accent consult audio, and its vocabulary, billing and letters are built on the systems you actually work with.
Internal benchmarks on synthetic Australian-accent clinical audio and public speech corpora. We publish our methodology — and we'd rather show you a flag than a wrong note.
The essentials, honestly answered.
An AI medical scribe listens to a consented consultation and drafts a structured note for you to review and confirm. On Free, your draft arrives after you stop; audio is kept until the draft lands, then deleted. A live transcript on screen is a paid feature. Uncertain and missing details still need your review.
Audio is deleted by default — Free deletes it when the first draft arrives; paid retains it encrypted for review and deletes it on confirmation by default; a practice can instead set immediate deletion or a short fixed retention window, and every mode sits under a hard 7-day ceiling. No. Small Mercy is a documentation aid, not a diagnostic or decision-support tool. It drafts from what was said, marks missing details [not stated], shows uncertainty and requires you to review and confirm every note before saving or export. As with any transcription or draft, errors can occur.
It surfaces uncertainty instead of hiding it: anything it wasn't sure it heard or couldn't place cleanly is flagged for your eye, and every note statement is grounded to the transcript. And its privacy is verifiable — live patient-facing checks and a QR deletion receipt, not just a policy PDF. Its clinical tuning is Australian today — PBS/AMH drug vocabulary and RACGP-aware letters — and we'd rather say that plainly than imply NZ-specific tuning that isn't built yet.

The web app, Windows and Mac desktop apps, iPhone and Apple Watch app, and Android app are all published. Clinical access is open in Australia, New Zealand and the United Kingdom. Free captures on web, iPhone and Android; paid adds desktop and watch capture.
New Zealand organisations use the AU cell. On paid plans, Cliniko connects for Australian-hosted accounts and Nookal through its Australian API service. Today’s appointments come into Small Mercy, and once you have confirmed which patient a booking is, you start the consult from it. Once you confirm the note and check the patient a second time, Small Mercy files it into Cliniko or Nookal as a draft treatment note for you to finalise there — and reads it back before it says it was sent. A confirmed letter can be filed as a PDF copy on the patient’s record; you still send the letter itself. Nothing is written until you confirm and send. Notes and letters also export as clean text and PDF.
